You've just reached the main gate of this medical electronic survey, which emphasize your full authority in using this medical surveying facility  for the 2^nd phase of "Saudi Project For Assessment Of Coronary Events"  [S.P.A.C.E Registry]  that will be available on the international web for the period (January.1^st To December.31^st ) / 2007.

In this survey, you are strictly required to complete all requested data for each involved patient before the submission of his (her) relevant CRF. If you are prepared with a pre-made hard copy, It will take approximately [2] minutes to complete the CRF.
